The Sketch symbolizes a Thought. You see a woman walking down a dark alley. It is clearly evident that something is troubling her: a thought, like a lightening bolt stuck in time forever. The surrounding paints her discomfort perfectly. The fog in the alley symbolizes her confusion. But we as observers don't know what she is thinking about. She seems to be a confident working woman. That's pretty obvious with her walking alone in the evening, the time when people usually head home after work. 


Her State of being could be compared to a mermaid stuck to the anchor of a ship.


So coming back to the question again. What is troubling her? We can, to an extent, only get to know how the thought feels, by looking at the sketch, but will never know what the thought actually is. Maybe it is equivalent to a mermaid being concerned of the litters in the ocean. So again we know how the thought feels, we know what it is equivalent to, but we still don't know what it is. The sketch isn't letting its observers form their own judgements about the thought because they are unaware of the thought. They just see how the woman feels about it. In a way the sketch doesn't care about what the people are thinking it is more concerned of the woman's point of view. It makes sense, as people look at a troubled person, ask what's bothering him/her, listen, compare it to their own life, and maybe don't feel sympathetic after knowing exactly what's troubling the person. Which is also very understandable. 

PS- Well 'Nope' is a reference to the Jordan Peele movie. The ship, the cloud or the thoughts are like the alien in the movie, constantly looming over people.
